Graceville Centre provides support to individuals living with a psychiatric, physical and/or intellectual disability. All Graceville services are conducted by working with individuals in a strength based and recovery focused framework. Graceville Centre operates seven programs that provide support to clients either on site or in their own homes.

We have an exciting opportunity for passionate and caring individuals to join our team of dedicated Support Workers. As a member of this fantastic team you will have the opportunity to work across various programs supporting clients to develop and maintain the lifestyle of their choosing.

We will provide all the necessary training to be successful and if required work with you and support you to obtain your certification. This is an area that we are passionate about and encourage people who feel the same to work with us to help make a difference.

Duties:

• Provide support to clients to enable them to reach their Individual Support Plan and Future Goals Plan
• Empower clients to live in the least restrictive environment and to access community activities
• Assist and support clients to maintain their health and well being including undertaking personal care
• Promote and role model positive behaviours, learning experiences and skills development
• Communicate effectively and professionally with all relevant stakeholders
• Complete, maintain and store documentation in a confidential manner
• Undertake other duties as required to meet operational needs.

Essential requirements are:

  • Excellent communication skills both written and verbal
  • Ability to work independently or as part of a team
  • The ability to make accurate and informed decisions in a timely manner
  • A willingness to learn
  • Possess a relevant qualification or be open to attaining
  • Maintain a current Drivers Licence
  • Be prepared to comply with the Disability Services Act 2006
  • Possess a current Blue Card or open to attaining
